All customers who make a minimum purchase of Php 1,500 in a single receipt are eligible to become members of the Healthy Options Green Card Loyalty Program. Membership is FREE.

2) When does my Healthy Options Green Card Membership expire?
The Healthy Options Green Card Membership has no expiration date. The Healthy Options Green Card however has a validity period. To renew the card, member must update contact information at any Healthy Options store on or before the expiry date written on the card.

3) What will happen if I lose or break my membership card?
A card fee of P 100 will be charged for the replacement of lost or broken membership cards.

4) Can a family member use my Healthy Options Green Card?
Membership of the Healthy Options Green Card is non-transferable, however family members can enjoy the shopping benefits (i.e. rebates, members-only discounts) if they are accompanied by the card holder.

5) What is a Healthy Options Rebate Voucher?
A Healthy Options rebate voucher is one of the privileges of Healthy Options Members. It is not for sale. The Rebate Voucher may be used to buy any product, including sale items from any Healthy Options store.

6) How do I earn a Healthy Options Rebate Voucher?
You must present your Healthy Options Green Card to the cashier in order to receive rebates. Every Php 1,000 single receipt purchase rewards you with a rebate voucher worth Php 50. Healthy Options will not reimburse retroactively for purchases made without a membership card. NO CARD, NO REBATE.

7) What are the mechanics of a Healthy Options Rebate Voucher?
The Rebate Voucher may be used to buy any product including sale items from any Healthy Options store.

It is not for sale.

No change shall be given for purchases of items less than the value of the Rebate Voucher.

The Rebate Voucher may not be exchanged for cash and will not be replaced if lost or stolen.

1. POINT SYSTEM
Each Healthy Options Member will be rewarded 1 point for every Php 10.00 spent on all items at any Healthy Options Store.

2. BENEFITS
Your Healthy Options Card Membership shall entitle you to:

• P50 Rebate Voucher for every P1,000 you spend in a single receipt.
• A complimentary copy of the Healthy Options News Digest published bi-monthly. Just present your Healthy Options Green Card at any Healthy Options store to claim your free issue.
• Earn up to 1,500 more points within ninety (90) consecutive days and automatically become a Healthy Options Gold Card Member. The Gold Card Membership will entitle you to more benefits including 10% discount on purchase for all cash and card transactions (excluding sale, promo and consigned items).
• Members-only discount on special merchandise offers presented throughout the year.

3. CERTAIN CONDITIONS

• You, as a member, must present your Healthy Options Green Card to earn points and enjoy the benefits of being a Green Card Member. Healthy Options will not reimburse retroactively for purchases made without a Membership Card..
• The rebate will be issued after all other deductions (e.g. discounts, redeemed Rebate Vouchers) are accounted for.
• Members will earn ten (10) points for every P100 purchase.
• Member's full name, address and telephone number must be accurately entered in the Membership Form to receive all the benefits. Members can change their personal information at any Healthy Options store.
• Membership is non-transferable.
• Membership is valid at any Healthy Options store.

4. FREE MEMBERSHIP

• Healthy Options Green Card Membership is given free to all customers with an initial P1,500 minimum purchase in a single receipt.

5. RIGHT TO CANCEL MEMBERSHIP

• You have the right to cancel your Healthy Options Green Card Membership at any time by notifying us at any Healthy Options store.

6. RENEWAL

• The Healthy Options Green Card Membership has no expiration date.
• The Healthy Options Green Card however, has a validity period. To renew the card, member must update contact information at any Healthy Options store on or before the expiry date written on the card.

7. LOST CARD & REPLACEMENT CARD

• A card fee of P100 will be charged for the replacement of Healthy Options Green Cards which have been lost or broken.

8. PRIVACY

• Member information will not be sold nor rented. Names and mailing addresses may however be traded and made accessible to partner organizations, but only upon expressed consent from Members.

9. REBATE VOUCHER TERMS AND CONDITIONS

• The Rebate Voucher is given free as a rebate to members. It is not for sale.
